Adding an Information/Instruction page to a form

Dear ODK Superiors,

Using XLS/ODK - What is the Syntax to create an Information/Instruction
page, that precedes a question page. In the XLS sheet, it should go in the
line above the relative question, and on the device should be seen as a
page in itself, before the question page.

See the Cells highlighted Red, where the Syntax needs to go:

Any thoughts greatly appreciated.

There is a 'note' field type that can be used to display information.

The example XLS form

several of these.

Dear ODK Users,

My devices have begun saving the forms on the device, even after they have
been sent.

I need the forms to be removed from the device, once the data collector has
sent the forms.

Can anyone tell me how to do this? Is it a setting in the Android ODK
device?

This is a bug in the delete-after-send logic. It will be fixed in the next
ODK Collect release.
