Sorry for basic question....Using ODK central I've created a simple form in excel to collect contact details of potential participants and uploaded this. Form works fine but I'm trying to add a logo and an information paragraph to the form and I can't seem to do this.
I've tried some of the previous work arounds posted on here a few years ago to put a note in but get the error message " Unknown question type 'Note'. "

The question type is case-sensitive. Does your form have it as an all lowercase note?

Will do. Thank you for your response. Is there a simple way to imbed a log in the form too?
You can add an image column with the filename of a the logo you want to include. The docs on it: https://docs.getodk.org/form-styling/#images
