I am trying to do a very simple table/matrix question in SurveyCTO which has 12 select_one questions in the rows and 3 columns: one for the question, and an additional two for other questions. Essentially I am trying to get responses for 2 questions for each month of the year. Table-list is working for me with only 2 columns, but how do I add an extra column for the additional question?

or select_multiple options -- where column 1 has questions and columns 2-x
have a common set of choices -- but not tables that also contain other
field types. So, for example, you can't have a table that includes
open-response text fields. We get a lot of requests for that and have it on
our roadmap to add, but we haven't gotten to it yet.

Just to clarify, I am fine with using a select_one field type in the table. What I'm trying to do is have three columns in the table, whereas the default with table-list is two columns (one for the question and one for the response).
(Also a side note to correct my error in the original post where I said field-list where I actually meant table-list).

Well, "field-list", combined with the "label" and "list-nolabel"
appearances, can get you where you want to go. Since I can't easily link
you directly to the help or sample form without your server name, please
take a look at the clip I took from the sample form's help topic, attached.
If you follow that method, there will automatically be one column for every
multiple-choice option.
Just to clarify, I am fine with using a select_one field type in the
table. What I'm trying to do is have three columns in the table, whereas

for the response).
