I have seen other people with this problem, but the only solution is to
give a developer right to the person assisting. I deleted and uploaded the
same survey in a short time frame, with "the database ended up with two
entries for it when there should only be one entry." If somebody could
help, that would be greatly appreciated. "This is a known issue with form
upload -- Google can kill the form upload process before it has completed
the replacement of the old form definition with the new form definition"
