Appearance="field-list" dont work

Hi all,
this code below simply does not put the fields on the same screen.
Why?
I'm using collect 1.1.5.

that is correct. multiple questions per screen is functionality that
will arrive in 1.1.6. that code is in trunk and has not been released.
