Building a table/matrix of text and numeric questions

1. What is the issue? Please be detailed.
I have been trying to use XLSForm to develop survey questions that include tables consisting of text and numeric inputs. I was able to create this for select questions but was unable to do so for questions that require text or numeric input.

2. What steps can we take to reproduce this issue?
Using the questions in the image below, create the questions using XLSForm

3. What have you tried to fix the issue?
I have tried to use groups and field-list appearance but these features cannot create those questions

Hi,

Not clear about your requirement. However, the form you shared is doable. You need to create nested groups and for a set of value use table-list attribute to group them together on a collect screen.
