I'm constructing a survey style form that scores 1 - 5 against a number of
criteria - if the score is 2 or less a second form needs to be called
allowing the user to input remedial actions, once the called form is
completed I need to revert to the original to continue the survey.
It is not currently possible. You have to put all the branches within
a single form. Our new tool "ODK Survey", now in alpha, will have
this capability. A beta release should be coming very soon.

Appreciate the swiftness of the response - is there a formal roadmap for
the ODK2 development branch?

One alternative approach is to put the questions from the sub-form
into the main form and use relevance to branch to it. Lots of reasons
why you might not want to use this approach, but if you don't want to
wait for Survey, it's your best option.
