I've often used codebook v1.1, successfuly; but, tis time, with a given form, I'm facing the following error message

"

The codebook app is very out of date and I'm not sure when I'll have time to fix the documented issues. Your best bet is to load the form in https://opendatakit.org/xlsform and print it.
