Converting XLS to XForm

1. What is the problem? Be very detailed.
Am getting this error message "Error: The survey sheet is either empty or missing important column headers." when I try converting an XLS form to XForm.
2. What app or server are you using and on what device and operating system? Include version numbers.
Am using windows operating system
3. What you have you tried to fix the problem?
I have not tried to fix it as I dont have an idea how to do it.
4. What steps can we take to reproduce the problem?

5. Anything else we should know or have? If you have a test form or screenshots or logs, attach below.

The survey worksheet has 3 mandatory columns: type, name, and label.Details explained here:

In addition to what @ARIF_AZAD_KHAN said, you also need to make sure that the worksheet name is survey. I often forget to do that and get this error. I've filed an issue at https://github.com/XLSForm/pyxform/issues/296

I have the same problem.
My sheet is correctly called "survey"
The three column headers Type, Name and Label are present
Still get the same error
Very frustrating

I fixed the labels, but have a new problem.
With KoboToolbox chosen as the server - I can submit forms without a problem.
However, with Google Drive chosen I get an error that reads:
"Upload Results
Unable to make API calls. If you are running a local build, please review the project readme"
Please help

That means you are not using the original ODK Collect app but your own fork right? If so please read: https://github.com/opendatakit/collect#using-apis-for-local-development

No, I'm not using my own fork or local build (at least that was never my intention).
I am using the KoboCollect app.
