As part of the [Access to Covid Tools (ACT) Accelerator](https://www.who.int/initiatives/act-accelerator), we have partnered with [FIND](https://www.finddx.org/) to create a ready-to-use form to help community health workers in collecting data when performing COVID-19 antigen rapid diagnostic tests (RDTs). These RDTs can detect the presence of the virus at the point of care and are a vital addition to the testing arsenal needed to contain and fight COVID-19. The form we have created allows both existing and new users to collect COVID RDT data in ODK deployments, either as a stand-alone tool or by incorporating it into an existing workflow. And to ensure easy data sharing, the form conforms with the ACT-A COVID Antigen RDT Data Model which was developed for WHO, Global Fund, and other partners.
