I am trying to create a table in my system but i was wondering can the developer include other options like "select_one none_mild_moderate_severe" instead of select_one yes_no only. Another thing please help me with codes (in excel format that i should use to do this table.

Hi there,
You can use select_one none_mild_moderate_severe if you have a choice
in your choices sheet with select_one none_mild_moderate_severe. If
this isn't working for you, send out a simplified form with one
question that shows the problem.
