Dynamic form title

Hi folks,

I'm building a form in XLS. I want a title which is based on responses in
that form. For example, a school survey will save the first form as
school_a, the second as school_b.

I believe this is done with the form_title option but that seems to require
the name of the xls file itself.

Thoughts?

Actually sorted it. I tried a direct link to a field but could only manage
to get it to work using the concatenate code as in the attachment
