Error: Problem persisting data or accessing data (SELECT * FROM CONSUMER_RAJAH_PHOTO_BN WHERE _TOP_LEVEL_AURI = uuid:ea356cf2-ceec-4fb5-8a85-462ca0b60c0a AND _PARENT_AURI = uuid:ea356cf2-ceec-4fb5-8a85-462ca0b60c0a is missing an attachment instance OR has extra copies.)

and once this is fixed, upgrade to the newest ODK Aggregate. See release
