H! !i i'm new in the group and also the new odk user,i have created xlsform
but when i upload it fails and this message comes 'The choices sheet must
have on the first row list_name and name', still i do not know what is long
with my form, i have attached my form together with this post, its hope i
will get help.

Hi Faraji,
You have a select_one question (Row 5) that references male_female,
but you don't have that choice list in the choices sheet.
