I am receiving an error that does not allow one of my phones to save. Seems
like only one phone.
Error is:
form save failed!: Attempt to invoke interface method 'int
android.database.Cursor.getColumnIndex(java.lang.string)' on a null object
reference
Any thoughts on what could be causing it?

This would indicate that there is a problem accessing the odk/metadata
folder.
First, try rebooting the device.
Verify that the phone has an external SDCard, and that you can open and
browse to the odk/metadata folder on that external sdcard (newer devices
have this under /storage/emulated...; older devices have this under
/sdcard...). This folder should contain the forms.db and instances.db
databases that ODK Collect uses.
If you have multiple user profiles defined on your device, this could be an
issue (we haven't tested behavior with those). E.g., you might have ODK
Collect open on 2 or more of those profiles.
