Hello, i want to create choose filter for region and district but getting errors: "The survey sheet is either empty or missing important column headers. Your file should have the .xlsx, .xml file extension because Ona uses XLS Forms to create forms."

It sounds like your XLSForm does not have a survey worksheet with the 3 mandatory columns: type, name, and label. You can read the documentation at:
