I found this from Waylon in the an older post and remember doing it (very
nice shortcut) but "shortcut" seems to be missing on my Android version
4.1.2. Any way to do this in newer Android versions?

Hey Curtis,
On newer Android versions this functionality is considered a widget. Go to
the "Apps" menu, where all your installed apps are displayed. This is often
a shortcut in the bar at the bottom of the home screen that looks like a
grid, although this may vary between devices.
At the top of the screen you'll see you can select either "Apps" or
"Widgets". Select Widgets and scan through them. The option you're looking
for is "ODK Survey Form". Select this and drag it to your home screen.
You'll then be able to select a form.
