Has anyone had any issues with uploading (blank) forms (with images) and then them disappearing?
We uploaded a form that 3-4 images linked to it. It was checked and working fine. Then when we came back to the form a few days later the images no longer showed up on it, although nothing had been done to the form on ODK aggregate etc.

If you have not already, you may want to restrict who has Form Management
and/or Site Admin access to your server.
