Inconsistent play of video media (mp4 and m4v)

I have programmed a survey in Formhub to play a video. I am using a samsung phone, media stored in device storage/odk/forms/survey media. The media files play infrequently. When they do not play, I receive the error message "File://storage/emulated/0/odk/forms/survey/- is missing". Has anyone else experienced a similar issue where the video files play in some instances but not others? There has been no change in the survey, phone, or files uploaded to the phone.

Is this on a single phone?
Or does it happen on all phones?
Is it for a specific video, or randomly across many videos?
Or is it randomly happening among a fleet of devices?

The first thing to check is that the filename of the video is correct in
your form definition, and that it does not contain any unusual characters.
E.g., just a-zA-Z0-9. When defining forms with translations, it can be easy
to inadvertently use a Unicode value that looks similar to one of these
characters, but would not be understood to be the same by the software.

The next thing to check is whether this is correlated with:

(1) use of multiple-user logins on a device. We have only tested operations
when there is a single user on the device. If you enable multi-user logins
on the device, there may be issues we have not uncovered.

(2) differing Android operating system versions.

This is on multiple (10) of the same make of phone, randomly across many videos. The file names are very simple, for example 12.m4v. There is only one login per device, but they are encrypted. All are running on Android 4.4.4.
