Invalid xml form

i have been trying to make this form valid with the ODK validator but
cant see where the problem is. Its probably something really simple!
I think new eyes are needed!

it comes up with "java.lang.NullPointerException" then a whole bunch
of other stuff!

very interesting.

in your select control, you have items that have no value. you have
encoded them as "" and that doesn't seem to work but
"" does. this probably points to a larger issue --
values probably shouldn't be blank.
