To capture time taken by the survey team I have included start, end, today question from metadata section.
Issue is start and end time for the first record is correct. For all next records the end time is taken to start time of the next record. See screenshot below

If I close the APP and start the entering data in form then it works properly.
When I add multiple surveys after other then the above issue occurs.

The times in your scan are not exactly the same. Even if short, this might be enough to close/save a case and at once load/start the next case.

start, end, and today rely on device time. Depending on the device hardware, operating system, clock configuration, network configuration, the device time can be wrong. To reliably measure the time between events (e.g., to know how long a survey took) or to get a more complete record of user behavior within a form, consider using form audit logging.
Also, even assuming device time is accurate, the
end time is last time the survey was saved. An enumerator or data manager can re-open and save the survey and it will look like it took much longer to fill out the form.
