The URL you should enter into your configuration is the URL that displays
the ODK Aggregate server webpage, without the webpage designation ( /
Aggregate.html#...)
Verify that the URL works within the browser on your device.
You might be hitting a wifi network login screen.

Thanks for the reply that u received. I have today managed to get a form from the server but when i am trying to fill it, it in the process brings me this notification "external data for the district_CPM has not been imported perhaps you forgot to include the district_CPM.CSV file with your form" therefore, I again begg for your help. Thank you
