Wasn't sure how to properly mark this as resolved, so I just edited the post.
Nevermind, I was able to resolve the issue. Even though my situation didn't match the instructions Mitch had provided exactly, I found out that it was because I had the original uploaded form along with an update that together seemed to be causing the issue.
