ODK Collect crashes when loading form, Please help

Hi,

When I download our form for first time it works well. But when I try to
load the same form again ODK Collect crashes. I attached our xlsx file.

Please help.

When I try load the Form entri, and then save, I loading angain, NO PROBLEM
with ODK on my phone

please chek your memory phone, maybe trouble with your memory, full or you
must cleaning first
