ODK Collect crashes when trying to submit forms?

Hi forum,

One of our applications of ODK Collect crashes each time we try to submit our completed forms. 3 other tablets and apps are working fine.

When we toggle one (or all) of the forms and hit "submit" the screen goes black and then a text box appears saying "ODK has stopped working."

Could really use advice for a work around on this. We've tried rebooting the device, rebooting the app.

We'll need a crash log before we can help. Use adb as described at
https://code.google.com/p/opendatakit/wiki/CollectTroubleshooting
