How to present such a questionnaire in odk

You can present this questionnaire by translating it into an XForm using ODK Build at https://build.getodk.org/, or an XLSForm in a spreadsheet program like Excel.
The docs at docs.getodk.org are detailed and helpful.
Have a go at that and see how far you get.
If you have specific questions about form design, don't hesitate to search the forum for possible answers, and if you can't find an answer, ask a question yourself with a detailed example.
Don't be afraid to refine the form many times until your enumerators are happy.
