There are select-one and select-multiple prompt types. You would want a
select-one.

If you are using select_one or select_multiple prompt, there might be the
case you give same name for two or more option. In case this is the reason
it cause duplicate selection if you select any one of the option.

All the gadgets have submitted the finalized form except 2 phones for the
past 24 hours. What could be the solution because I've tried all the
possible troubleshooting?
