I have not found in any post the answer to this question:
when I finished the collection of data on my android device before sending to
the server, I should print a report of the form so that a person may affix a
signature for receipt. How can I do this?

You might want to use our Signature Capture widget (
http://opendatakit.org/help/form-design/examples/#image_prompts )
To have them sign their signature and save that within the form itself.
Otherwise, if you must get a hard copy, it would be very difficult to do
with a large form -- you would have to use a printer such as
