Problem with ODK Collect when forms were instaled on different devices-sunsang tab note and sunsang smart phone

Hi,
i have another problem--the problem with ODK Collect when forms were installed on different devices-sumsang tab note and sumsang smart phone Galaxy grand prime.
It works will with the Sumsang tab note but when with the later, the options of answers to two for two of the last questions are mission.
could you let me know what is the problem?

The most likely causes (in order) are:

(1) you are trying to display a set of questions on a single screen where
the last two questions have a relevant condition or choice list that
depends upon the answers to the first two questions. Swiping forward or
backward from this screen and returning to it should correct the display.
This is by design and is not a bug. You are not allowed to have questions
that have relevant conditions or choice filters that depend upon other
questions on the same screen.

(2) you have two similar but different form definitions sharing the same
form id. Verify that the form definitions are exactly the same on the two
devices.

(3) you are running different versions of ODK Collect on the two devices.
Verify that the version shown on the opening screens are the same. The new
release is ODK Collect 1.4.5 (1048); it began being distributed to devices
on Monday.

If (3), then your form has some logic that is evaluated differently across
the two ODK Collect versions. If the newest version does not operate
correctly, please create an issue describing the problem

Surprisingly I replaced the Collect with the latest version, then the problem was solved automatically. But thanks so much for providing in-depth analysis on potential problems.
