Hello,
I have been trying this from 5 hours but it's not fixed yet.
The problem is- some relevant conditions accepts name field but some does
not. Like in attached file line 19 and 20 gives error but 18 and rest of
the relevant does not.

First, please, try 'completed' instead of 'Completed'.
Yeah I tried that, don't know why but when I added _status (eg.
demoliton_of_house*_status*) on every name, it worked.

Hey there!
It might sound funny but may I suggest you open a blank Excel and copy each
sheet from the current excel to the blank one and try your form
again...once I had similar problem and everything checked out and it still
did not work then I tried that and it worked....

Yeah sometimes weird happens in xls forms. I have faced that too.
