Problem with the Get Blank Form when I created a new different ODK Aggregate Account!

I've in use an ODK Aggregate account which is known by my phone, but once
I've tried to create a new ODK aggregate account and I've tried to change
URL site, replacing with the new created, it appeared and required me to
fill in the username and password and I keep doing so but it still keep
requiring me to do that again. I don't know what is the problem. Just want
to note that I have successfully created the new ODK aggregate, I can log
on to that new site of ODK aggregate and I also can publish the built form
into the account as well as I'm quite sure that I use my address correctly
when ODK asked me to fill the username and password of the server in the
phone.
However, If I'm not wrong, it is not necessary to fill the username and
password of our server into the phone. Why once using the second account it
required the address of server?

I'd be appreciated any answers and I'd to express my grateful for the
answers.

ODK Aggregate requires ODK Collect to authenticate (log in) using an 'ODK
Aggregate' username and password.

On ODK Aggregate's Site Admin / Permissions page:
(1) enter a username in the box at the bottom of the page, and click
'Add'. The username will appear under the list of users with an 'Account
Type' of 'ODK'.
(2) Check the 'Data Collector' checkbox for this username and click 'Save
Changes'. This grants the user the ability to access the server from ODK
Collect.
(3) Click 'Change Password' for this username and enter a password for the
username, and click 'Change Password' to apply that change. Until a
password is set, the username will be unable to access the server.

On ODK Collect, click the 'Menu' button (or 3 small vertical bars), then
click 'General Settings':
(1) change the server URL to be the URL for your ODK Aggregate site. For
Google App Engine accounts, this generally is of the form: *
https://your-app-id.appspot.com* note the 'https' prefix, and the lack of
the /Aggregate.html at the end.
(2) also enter the ODK Aggregate username and password you created above.
If you do not enter the username and password here, you will be continually
prompted for them.

Unless you enter the username and password via the 'General Settings' page,
ODK Collect only remembers them for a few minutes, after which it will
re-prompt for them. If there is an error during the connection to the
server, it may also clear usernames entered in this manner and re-prompt
for them.
