Hello Matt,
What did you find out about this crashing behavior? We have a LAMP server
and are experiencing this same thing with Collect 1.46 and 1.47 (not 1.45).
I have yet to make a stack trace on what android is doing when it dies
(seems to be independent of Android version).

There also seems to be a very large spike in Out-of-Memory errors with
1.4.6 during the reading of a form definition. Looking into the
OpenStreetMap changes to see if anything might have caused a problem
there...

Emptied out the forms folder and tried 1.4.7 (1053). Still crashed when
asked for form list, but this time after the "OK" crash message, it brought
me back to the Collect main menu instead of totally crashing the app.

Without a stack trace, there is zero chance of fixing this.
If you cannot provide a stack trace, try deleting the .cache directory.
That might help, or maybe not. But it won't help fix the issue.
Note that the 1.4.7 release was not a "stab in the dark". It has changes
that were identified from stack traces people supplied and from Google Play
crash reports.
