Hi Moteb, I hope that you are well.
In the csv file you attached the id_key field matches the "label" column in the choices tab of your form. It needs to match the "name" column. Everything else in your form looks good and the pull data function seems to have no errors.
I hope this helps. If it still doesnt work for you let me know and I can modify your example. The easiest way to fix it is probably just to make the names and the labels for the "id" choices the same.
