Replace forms

I have a form with around 50 fields and I have been collecting data for
past 2 months
I now want to make a few changes to the form by adding around 4 fields to
the xml form, meaning a new form
Dilemma - these two different forms will create two different tables in
MYSQL hence merge in stata during analysis.
*Qtn. *Is there a work around where I can have both new and old form data
be in the same table

Short answer, no. Long answer, use derived tables or views.
