Hello ODK Community,
I am creating a questionnaire to rate the quality of health services where
different criteria of services are given values of 1-4 so that is an easy
select-one question.
After each question/criteria, however, there is a space for
observations/comments to justify the rating on the criteria. How can I add
a text question so that the comment is linked to the specific criteria?

I don't understand the question.
All of the fields/questions are uniquely named and present in the data
table / CSV produced by ODK Aggregate or exported by ODK Briefcase.
So if you have
criteria_1_rating
criteria_1_justification
...
criteria_4_rating
criteria_4_justification
you would have 8 columns in your data table that you would analyze.
