I need to show image on the basis of calculation.
e.g. if result is <21 the will show "low risk" image
if result is between 21 to 32 then "Moderate risk" image
and greater then 32 then "High Risk"
How I can achieve it in the XLSForms?

Use relevance (http://xlsform.org/#relevant) to show different prompts
based on previous answers. Once you get that working, then change
those prompts to include images (http://xlsform.org/#media).
