Single select shows 2 options selected

Hi?
I have seen single select questions that display 2 selected options when
you return to the question before. How ever is only shows that one has been
selected when viewed in the quick prompt.

Is this an issue that can create data entry errors (double entry for single
select options)?

What version of Collect are you using? Is the problem reproducible?
Can you make a 1-2 question form that shows this problem?
