I am having a rather bizarre problem with a form that is hosted in FormHub. A series of three questions are not appearing on some users devices, while on others they are appearing. There is no "relevant" constraints and the questions are not in a group. The questions are also required. The basic logic of each question is very straightforward, and basically look like the XLS form attached. Has anyone seen this happen before? Any idea what caused it?

What versions of ODK Collect are your devices using?
What versions of Android OS are they using?
Are these all the same device, or different?
If you install the same forms on a corresponding device, does it work
(i.e., is it specific to a particular device with the same ODK Collect and
Android OS, or does it occur on all devices with a similar configuration.
