First, thanks so much for answering many recent questions!
Some of us have been working on getting an up-to-date XLSForm with all
widget and appearance combinations. See the last comment at
https://github.com/opendatakit/collect/issues/272 for a Google sheet
and refer to rows 47 and 48 for the OpenMapKit integration.
I must admit that I don't really know how to use OpenMapKit and I
believe it needs a server to connect to in order to do anything
useful. All I can really confirm is that the app successfully gets
launched. Is that enough to get you going or are you also looking for
information on how to use OpenMapKit in combination with Collect?

Were you able to do what you needed to do with the tool? As I was looking around, I came across some really good workflow documents that Dan and others at the American Red Cross maintain. You may be interested in checking out this one in particular.
