XLS Forms not recognising groups

Hi all,

I've put together the attached form in ODK build, and have uploaded it to
Aggregate (using the Form Manager inside Aggregate). However, when I access
it on the tablet the group function doesn't work (all of these questions
should be on one page). Does anyone know why this is the case?

Groups are used for both managing relevance and for controlling appearance
(e.g., all questions on the same screen).

To get all questions on the same screen, in the begin group line, you need
to specify* field-list* as the appearance.
Otherwise, the group is only used for managing relevance.
