XLS2XForm Conversion Error

we're experiencing an error in XLS2XForm for the first time that reads

Conversion Error: ('Element with this name already exists.', [u'

followed by a list of some of the variable names.

has anyone had this same error? I could not find anything on the same
issue on the ODK Community.

Hi Andrew,
Sorry for the delay. I am attaching our xls file now (ending with
110917md). Also we were able to convert an earlier and much shorter version
of the same questionnaire. I am also attaching it (ending with 110831md)
and the corresponding xml.

Thank you for your assistance. I was able to convert the file and we will
continue to program the skip patterns, etc...
