Not sure what you're configuration is. A quick way to debug this sort of
thing is to open a browser and go to the URL (
http://localhost:8888/formList ). It should show an xml document listing
the forms available for download. I.e., with 2 forms, the content is
something like:

Riser Inspection Adult Module
