Annotate media embedded in form

Hi,

I'm designing in Excel and wondering if it is possible to embed an image
which you can annotate? I understand that those can be done separately but
wondering if it is possible to combine them.
I have attached my survey & you can see in Row 40 what I am trying to
accomplish.

An intriguing idea. I've created this issue:
http://code.google.com/p/opendatakit/issues/detail?id=726

with a summary of the changes needed to implement this.The current Annotate
Image widget does not support this, but it would be a fairly easy code
change to create an annotate mode that would accomplish it.

Code contributions gladly accepted.
