Audio Not Showing up Properly for Questions or Choices in WebForms

1. What is the issue? Please be detailed.
Audio shows up properly for questions and choice list in a selects question in Enketo (Although it doesn't show up properly for a choice list that has been filtered). All audio shows up properly in Collect. In WebForms, I don't see any buttons or clicking the responses doesn't play the audio at all.

2. What steps can we take to reproduce this issue?
Upload the attached XLSForm and Media to ODK Central and view the form in Enketo, Collect, and WebForms.

3. What have you tried to fix the issue?
Tried Several Different Types of appearances for select multiple question and several different question types.
