fully-supported product based on ODK for the commercial market.
"doForms" works with a wide selection of Android and Blackberry
devices. Versions for the iPhone and iPad are scheduled for later in
the year.
doForms is absolutely free for 1 mobile device; with multi-device
plans starting $30 per month. Grants and discounts are available for
students, teachers, educational institutions, and non-governmental
humanitarian assistance and environmental organizations.
