The xls version you attached worked for me but I used the online version. It's very likely that the one you are using is too old and doesn't recognize those new parameters.

With the audit file you have attached, how to know if this is for a particular survey? As there is no "UUID/Instance_ID", how we can specify, as multiple surveys will be conducted on the same form and in same day?
