Data incompatibility

Hi everyone. Am posting a problem for the first time
I built an odk form and the data received on my google drive(server) is not compatible.
Example where the question is a placeholder button question of Yes or No. When the field person select Yes. ODK submits no to the excel sheet. And where I.d is 004 it submits Area Code of LGI (Lagos Island). Help me figure out the error

could you be a bit more specific because I'm afraid I don't understand well where the problem is.
This is a form you have sent from ODK Collect to Google Sheets right?
Could you attach your form?

Could you please share your xls form. You may remove all parts and keep the lines you are experiencing issue with.

Thanks i abandoned that already
I built another one on the aggregate but i could not login nor upload a new form on the aggregate.

My password and username is not working
Can u help me with this one.
More important now

Hi,

Your user ID should be the ID you defined or administrator for DO and default password should be aggregate.
