Data lost from saved form

Hi All,
I am running into a glitch that I do not know how to trouble shoot. When i
fill out a form, finalize it and save it and then proceed to access and
edit it at a later date (on my tablet) there are certain questions whose
answers have been deleted. This does not happen to all the questions, only
a few. I attached the XLS form. Any help or insight would be greatly
appreciated.

Hi Sarah,
is the data not saved (i.e. when you download the instance is the data also
missing?)
-or-
is it not displaying in the edit mode? (i.e. it looks like it is missing
while editing, but when you download the instance all variables are
populated?

I've seen something similar when you have multiple versions of the same
form on one device - I think the error is 'multiple definitions of the same
form'. For example, you have one form, you edit it a little, rename some of
the variables and put it on the device as well...then the data really does
NOT save.

I have also found that when you use an itemset with choice labels that are
composed of two words, when you go to edit, it looks like the data is not
there, but when you download the instance it is all saved. Since that was
confusing for our data collectors, I swapped the space with an
underscore.

Another thing to understand is that data from prompts that become
irrelevant are not saved.

That is, if you take branch A, save the form, edit the form and decide
to take branch B instead, all the answers in branch A will be thrown
away on save.
