I have a survey that requires each third participant registered on the survey, must be identified and enrolled into a substudy. There is any way that ODK can help me to automate such activity?

Does this need to happen on the spot - that is, will registration and the substudy questions occur during the same interview? Or, does registration occur and then you later go back to interview the people selected for the substudy?

Yes, it is supposed to happen on the spot after each registration the third participant must be identified for the substudy.

Maybe something like this:

The form contains an additional group of questions that are displayed only for every third group (in your case it might be participant registered).
