Enketo failed to load form

1. What is the issue? Please be detailed.
My ODK account was setup last month through a University system. I uploaded my xlsx form about 3 weeks back (was running perfectly well in enketo) and would now like to use it to collect data. Unfortunately, when I load it again via the web browser, I get an error as in the link https://odkcentral.pps.wur.nl/-/preview/ObsnGvr1sVhU15dwT5dHnbMD9mLomwU (screenshot below). I am new to ODK and have no crew of what could be the problem or even how to go about it. This is to kindly seek your assistance; thanks.

2. What steps can we take to reproduce this issue?
No idea, maybe to try and upload an xlsx form and see if it loads via the web browser.

3. What have you tried to fix the issue?
I deleted all the forms and uploaded a new form but the error persists.

the only strange thing I see on your form is that in the settings you have form-id instead of form_id.

I tested your form on my ODK central (version 1.5.2) and it works smoothly.

I see that you are on version 1.5.0, maybe a bug on that version of Central.

I would update to the new version, I see that on 1.5.1 there was an update on Enketo.
